LANGUAGE ACCESSIBILITY & NONDISCIMINATION NOTICE
The Care Team (TCT) is committed to complying with Section 1557 of the Patient Protection and Affordable Care Act by providing services to eligible clients and does not discriminate based on race, color, national origin, age, disability, language or sex.
TCT will provide the following free aids and services to individuals with disabilities to enable them to communicate effectively:
1. Qualified language interpreter
2. Qualified sign language interpreter
3. Written information in other formats including large print, audio and video formats, as well as others that are necessary and feasible
4. Written information provided in languages other than English
